Definitions

  • Health Equity1
    • The attainment of the highest level of health for all people, where everyone has a fair and just opportunity to attain their optimal health regardless of race, ethnicity, disability, sexual orientation, gender identity, socioeconomic status, geography, preferred language, or other factors that affect access to care and health outcomes.
       
  • Population Health Sciences
    • The study of health outcomes, including underlying causes and trends, within a defined group of people and the subsequent development and implementation of data-driven initiatives designed to improve the health status of the group.
       
  • Population Health Research
    • The application of scientific methods to determine 1) the underlying causes of health outcomes within a defined group of people and/or 2) the impact of health interventions on the health status of a defined group of people. 
      • Examples of types of research and study design methods: Applied clinical research, health services research, observational studies, epidemiological studies, quantitative research methods, qualitative research methods, mixed-methods
         
  • Population Health Management
    • The strategic application of a set of population health initiatives designed to achieve a vision and set of goals to improve the health of a defined group of people or groups of people, typically focused on improving quality and achieving value in the healthcare delivery system
       
  • Population Health Initiative
    • A program, policy, or project designed to improve the health outcomes of a group of people by addressing a health risk (i.e. obesity), health behavior (i.e. smoking), illness (i.e. influenza), disease (i.e. diabetes), social or structural determinant of health (i.e. healthcare access, food access, community safety, transportation).
       
  • Population Health Practice
    • Interprofessional teams, community members, and organizations/entities working together to improve the health outcomes of a defined group of people.  
      • Examples of organizations/entities: Healthcare Organizations (Hospitals, Health Centers, Physician Groups, Insurance Companies), Government Agencies, Civic/Citizen Associations, Faith-Based Organizations, Community-Based Organizations, Social Support Agencies, Institutions of Higher Education, K-12 Schools, Volunteer Health Organizations, Trade Associations
  • Community Engagement
    • The collaboration between institutions of higher education and their larger communities (local, regional/state, national and global) for the mutually beneficial exchange of knowledge and resources in a context of partnership and reciprocity.  The purpose of community engagement is the partnership of college and university knowledge and resources with those of the public and private sectors to enrich scholarship, research and creative activity; enhance curriculum, teaching and learning; prepare educated, engaged citizens; strengthen democratic values and civic responsibility; address critical societal issues; and contribute to the public good.
